چکیده

AIMS Ramipril and candesartan have distinct mechanisms of action to improve endothelial function. Therefore, we hypothesized that combination therapy has additive beneficial effects to simultaneously improve endothelial dysfunction and adipocytokine profiles in patients with hypertension. METHODS AND RESULTS Thirty-four patients were given ramipril 10 mg and placebo, ramipril 10 mg and candesartan 16 mg, or candesartan 16 mg and placebo daily in a randomized, double-blind, placebo-controlled cross-over trial with three treatment arms and two washout periods (each 2 months). Ramipril, candesartan, or combination therapy reduced blood pressure, improved flow-mediated dilation, and increased plasma adiponectin levels when compared with baseline values. However, combination therapy improved these outcome measures to a greater extent than either ramipril or candesartan alone (P < 0.001 and P = 0.016 for systolic and diastolic blood pressure, P < 0.001 and P = 0.048 for flow-mediated dilation and adiponectin levels by ANOVA). In addition, combination therapy reduced plasma leptin levels to a greater extent than either ramipril or candesartan alone (P = 0.042 by ANOVA). There were correlations between percent changes in adiponectin levels and percent changes in insulin sensitivity (determined by QUICKI) (r = 0.319, P = 0.066) following ramipril therapy, percent changes in QUICKI (r = 0.374, P = 0.029) following combination therapy, and percent changes in QUICKI (r = 0.607, P < 0.001) following candesartan therapy. CONCLUSION Ramipril in combination with candesartan improves blood pressure, endothelial function, and adipocytokine profiles to a greater extent than monotherapy with either drug in hypertensive patients.

Beneficial effects of pioglitazone on hypertensive cardiovascular injury are enhanced by combination with candesartan.

The effect of pioglitazone, a peroxisome proliferator-activated receptor gamma agonist, on hypertensive cardiovascular injury is unknown. We examined the effect of pioglitazone on hypertensive cardiovascular injury and the significance of combination of pioglitazone with angiotensin type 1 receptor blocker. Stroke-prone spontaneously hypertensive rats (SHRSP) were orally given pioglitazone, can...

Role of renin-angiotensin system blockades in reciprocal relationship between insulin resistance and endothelial dysfunction.

Role of Renin-Angiotensin System Blockades in Reciprocal Relationship Between Insulin Resistance and Endothelial Dysfunction To the Editor: Azekoshi et al1 demonstrated that the enhanced production of angiotensin II by free fatty acid in mononuclear and polymorphonuclear cells caused activation of leukocytes that consequently impairs endothelial function. Obesity, metabolic syndrome, and type 2...

Distinct metabolic effects of different classes of antihypertensive drugs.

Distinct Metabolic Effects of Different Classes of Antihypertensive Drugs To the Editor: Eriksson et al1 investigated insulin action and secretion and body fat distribution after treatment with candesartan, hydrochlorothiazide, and placebo in 22 hypertensive patients. They found that blood pressure was reduced similarly by candesartan and hydrochlorothiazide versus placebo after a 12-week treat...

Additive beneficial effects of fenofibrate combined with candesartan in the treatment of hypertriglyceridemic hypertensive patients.

OBJECTIVE Mechanisms underlying fibric acid and angiotensin II type 1 receptor blocker therapies differ. Signaling from peroxisome proliferator-activated receptor alpha may cross-talk with the angiotensin II system. We investigated vascular and metabolic responses to these therapies either alone or in combination in hypertriglyceridemic hypertensive patients. RESEARCH DESIGN AND METHODS This ...

Cardiovascular effects of combination of perindopril, candesartan, and amlodipine in hypertensive rats.

The combination therapy with ACE inhibitors, angiotensin II type 1 (AT(1)) receptor antagonists, or calcium channel antagonists may exert more beneficial effects on cardiovascular diseases than monotherapy. Perindopril, candesartan cilexetil, or amlodipine alone or the combination of low doses of each agent was administered orally to stroke-prone spontaneously hypertensive rats (SHRSP) for 4 we...
